Care Coordinator - St. Andrew's Nursing Home (Buangkok), Singapore

Key Responsibilities Provide support and assistance to Care Staff in the delivery of programs to address the psycho-social and mental needs of the residents under the supervision and direction of trained nursing staff. This job role includes providing support and assistance to Registered Nurses (RN) and Enrolled Nurses (EN) in the delivery of care services, including the maintenance of a clean, orderly, safe ward environment and the performance of care functions under the supervision and direction of trained nursing staff. General Duties - Conduct meaningful programs for residents according to their interests on a weekly basis. - Conducts routine exercise for the residents. - Track and monitor improvements of resident's mental and psycho-social status with indicative tools. - Collate holistic care plan from the professional teams (doctors, nurses, etc.) and incorporating programs to address the psycho-social and mental needs of the resident. - Assist in communicating with NOKs and liaising with hospital. - Alert the RNs/ENs to any change in the residents' behavior and medical conditions.
Care Delivery - Complies with approval standards of nursing practices. Ensure the safety, comfort and well-being of the residents and reports the residents' needs to the RNs/ENs. - Assists the Care Staff to attend to the needs of the residents, including serving meals and drinks, feeding, transfer and positioning of non-ambulant residents, personal hygiene and vital signs taking. - Maintain accurate documentation of care given. - Accompanies residents for medical appointments and outings. - Carries out any other duties assigned by RNs/ENs.
