• Up to 3 months of training, including job shadowing.

• 5 Days work week, regular office hours.

• Assess patients conditions and implement nursing care plan.
• Perform nursing procedures in patients home, such as: wound dressing and management, removal of stitches, and dispense medication.
• Attend to patients medical needs and provide a high standard of care through health assessments, planning, implementation and evaluation of nursing care to meet the needs of the patients.
• Design a holistic care plan for patients.
• Work closely with a diverse group of partners, as the care delivery required may not only depend on just nurses.
• Provide home care services for patients transiting from hospital to home.
• Promote healthy lifestyle education, including educating the carers of the patients.
• Participate in community health lifestyle activities.

• Candidate must possess at least a Diploma in Nursing with valid AED/BCLS
• Registered nurse with the Singapore Nursing Board, with a valid Practising Certificate
• Ready to coach, lead and supervise junior nurses
• Good communication and interpersonal skills
• Independent, reliable and not afraid to talk to people
• Strong sense of integrity, cheerful and pleasant
• Able to work independently and with a team
• Prior experience working in the industry will be an advantage
