Description
The Project Lead (PL), Community Care spearheads a three-year initiative aimed at strengthening caregiver support within the community. The PL oversees respite care operations at Tzu Chi's AACs and in-home settings, leads a multidisciplinary team, drives stakeholder partnerships, and ensures high-quality service delivery aligned with Tzu Chi's humanistic values. The role includes programme design, staff supervision, service evaluation, and continuous improvement to uplift caregiver and care recipient well-being.
Employment Type Contract of up to 3 years, with option to convert to permanent position based on performance
Key Responsibilities
Service Operations and Oversight i) Lead, plan, and manage centre-based and home-based respite care services. ii) Ensure operations adhere to established Standard Operating Procedures (SOPs), safety requirements, and regulatory guidelines. iii) Support caregivers by demonstrating basic caregiving techniques and responding to emergencies within defined competency levels. iv) Maintain proper documentation and case records. Team Leadership and Supervision i) Assign duties, supervise frontline staff, and provide coaching and guidance. ii) Conduct periodic reviews to ensure service quality, adherence to SOPs, and staff competency development. iii) Foster a positive, collaborative, and mission-driven team culture. Stakeholder and Partnerships Development i) Build and sustain partnerships with stakeholders including hospitals, community organisations, government agencies, and corporate partners. ii) Coordinate referrals, collaborations, and cross-agency initiatives to support caregivers and care recipients. Program Development i) Co-design and curate structured programmes for both care recipients and caregivers (e.g., functional maintenance, caregiver empowerment sessions, respite activities). ii) Work closely with AAC teams to integrate services into broader community care offerings. Monitoring, Evaluation and Reporting i) Track programme outcomes, service KPIs, and operational efficiency. ii) Lead service improvement initiatives based on data insights, staff feedback, and caregiver needs. iii) Conduct caregiver feedback surveys and translate findings into actionable improvements. iv) Submit periodic reports to management and relevant partners in a timely manner.
Qualifications & Skills
Diploma/Degree in Social Work, Nursing, Counselling, Community Care, Gerontology, or related fields. At least 3–5 years of experience in community care, eldercare, or social services. Experience supervising staff or leading programmes. Strong interpersonal, organisational, and communication skills. Ability to respond appropriately to emergencies and support basic caregiving techniques. Comfortable working in community settings and conducting home visits.
