Occupational Therapist (Nursing Home, Upper Thomson), Kampong Ubi

Provide high-quality, professional physiotherapy services across assigned caseloads, including inpatient, outpatient, and home-based cases. Ensure service coverage for designated wards. Conduct and document all aspects of patient care, including initial assessments, periodic reassessments (every 6 months for maintenance and every 3 months for active rehabilitation), individualized rehabilitation goals, weekly review of rehabilitation plans, treatment updates, and discharge planning. Monitor and follow up on patient progress. Participate in case conferences, multidisciplinary team meetings, and family meetings. Implement appropriate safety measures to protect patients during rehabilitation, including conducting risk assessments of the clinical environment. Communicate effectively with patients and their families to support rehabilitation, reinforce skill application, and promote therapeutic carryover beyond therapy sessions. Ensure the safe use of all equipment and promptly report any defects, incidents, or complaints to the appropriate authority for resolution. Actively engage clients, families, and caregivers in treatment planning, reviews, and rehabilitation processes. Provide safe and professional caregiver training to family members or carers when required. Utilize evidence-based, standardized assessments and interventions to assess, plan, implement, and evaluate care. Monitor, evaluate, and review Occupational Therapy treatments within a multidisciplinary framework, making necessary adjustments to optimize outcomes.
